Our Doctors

Dr. Krivacic attended University of Texas and The University of Houston. He received his Doctor of Optometry degree in 1981 and MBA from Texas Tech University in 2011. He is a member of the American Optometric Association, the Texas Optometric Association and the North Texas Optometric Society. Dr. Krivacic is the current president of 1st Eyecare and long time member and treasure of the Irving Sunrise Rotary Club. He has also participated in the initial FDA studies for laser surgery. Dr. Krivacic’s hobbies include playing tennis and golf, as well as traveling. He enjoys spending time with his wife, Paige, and their English Bulldogs.

After seeing Macular Degeneration change my grandmother's life, it has been my goal and mission to care for and preserve my patients' vision - a sense that we often take for granted. How are you helping patients live their best lives? I strive to provide my patients with the highest quality of eye care, the latest technology and resources, and educating them on the importance of healthy habits so that their vision can be the best possible. What does providing a transformational eyecare experience mean to you? Ensuring that my patients are happy and satisfied. How are you impacting your local community? I hope to educate the public on the importance of regular eye exams, volunteer at vision screenings, and take part in mission trips abroad. Hobbies/Interests: Exercising, traveling, and spending time with loved ones, family, and friends.

Dr. Gina Kubesh brings warmth, dedication, and a fresh perspective to St. John’s Eye Associates. Born and raised in a small town in western Maine, she learned early on the impact a caring optometrist can have. Inspired by her own childhood eye doctor, Dr. Kubesh set her sights on providing the same kind of trusted, patient-centered care that once made such a difference in her own life. Dr. Kubesh earned her Bachelor of Science degree from the University of New England before completing her Doctor of Optometry at Nova Southeastern University. During her studies, she was recognized for her academic achievements as a member of Beta Sigma Kappa. Dr. Kubesh provides routine and comprehensive eye exams, contact lens fittings and management, the diagnosis and treatment of common eye conditions, including eye diseases, and pre- and post-operative care. With roots in a community where healthcare was often limited, Dr. Kubesh values the role of an optometrist not just as a clinician, but as a trusted presence in patients’ lives. She is committed to building long-term relationships with patients, offering care that is as genuine and personal as it is clinically excellent. She enjoys seeing a variety of patients and helping each one feel confident in their vision and overall eye health. After graduating, Dr. Kubesh moved to St. Johns with her longtime boyfriend and their cat, Iris. When she’s not caring for patients, she loves being outdoors, whether hiking the mountains of her native Maine or enjoying Florida’s beautiful beaches. She also enjoys golfing, reading, and painting.

By providing excellent primary eyecare to patients for 38 years. Hobbies/Interests: Fishing, golf, poker. Spending as much time as possible with two wonderful grandchildren.

Certifications/Awards: President of Volunteer Optometric Services to Humanity (VOSH) during the third year of optometry school. Hobbies/Interests: Cooking and antique/thrift shopping.

Adalina Kulbak is an optometrist with thirty years of experience in eye care. Adalina received her Bachelors in Science at Loyola University in Chicago, Illinois before attending Illinois College of Optometry where she would earn her Doctrine of Optometry. Her experience has cultivated her comprehensive understanding of family and individual ophthalmologic screening, fitting, and disease management. Her most recent endeavor, small business ownership, taught her the balance between examination, diagnosis, treatment, and the marketing of additional services including cataract, glaucoma, macular degeneration, diabetic retinopathy, and dry eyes. She is fluent in English, Russian, proficient in Hebrew and Polish. Dr. Kulbak lives with her husband Mark, and her dog Charlie and has two grown children.

By ensuring that all my patients receive thorough comprehensive exams and providing them with the necessary recommendations to see their best. What does providing a transformational eyecare experience mean to you? Ensuring the happiness and satisfaction of all patients. Hobbies/Interests: Spending time outdoors with my family.

Dr. Lauren Kurowski is an Arizona native and recent graduate from Arizona College of Optometry (Midwestern University) in 2023. While in school she was a member of the Beta Sigma Kappa Honor Society for academic excellence. She achieved clinical honors while completing externships in primary care, ocular disease and pediatrics. Outside of the office, she enjoys spending time with her husband and baby girls. She enjoys traveling and being outdoors.

Dr. Reshmi Kurup is a dedicated optometrist with a passion for improving vision and eye health. She completed her undergraduate studies at SUNY Binghamton in the selective 3/4-year joint degree program with SUNY College of Optometry, earning her BA in Biology in three years. She then earned her Doctor of Optometry degree from SUNY College of Optometry, where she received extensive training in ocular disease, primary care, and specialized treatments. Dr. Kurup completed her residency at the Hudson Valley VA, focusing on ocular disease and primary care. She later achieved Fellowship status with the American Academy of Optometry (FAAO) by meeting rigorous requirements, including residency completion, peer-reviewed case reports, a published poster, and passing an intensive oral examination. Prior to MyEyeDr., Dr. Kurup has worked for several years at a solo optometrist practice, building a loyal and growing patient base. Outside of optometry, she enjoys dancing, creating art, and traveling with her husband. She is committed to providing personalized, compassionate care to all her patients, ensuring their eye health and well-being are always a priority.

Undergraduate Education: University of Maryland-B.S. Psychology, B.S. Zoology. Optometry School: Pennsylvania College of Optometry 1982. The reason I became an optometrist is: Help my fellow man overcome their visual problems. My favorite part of helping my patients is: optimizing their lives thru seeing their best. In my spare time, I enjoy: Travel, working out and spending time with my dogs.

Dr. Bhagyashree Lad is a Therapeutic Optometrist and a certified Optometric Glaucoma Specialist. She received her Bachelor of Science degree in Biology and Health from the University of Houston. She then went on to continue her Doctorate of Optometry degree at Rosenberg School of Optometry in San Antonio, Texas. Dr. Lad completed her externship at Eye Center of Texas focusing pre- and post-operative management of cataract and other refractive surgeries, treating and managing ocular disease, Glaucoma, and retinal pathology. She also completed her rotation at VA Outpatient Clinic in Jacksonville Florida where she concentrated in primary care, fitting contact lenses and low vision. Dr. Lad enjoys spending quality time with family, traveling and trying new foods.

Undergraduate Education: University of California, Santa Barbara. Optometry School: Southern College of Optometry. Honors/Membership: Beta Sigma Kappa International Optometric Honor Society, Gold Key International Optometric Honor Society. Hobbies/Interests: Cooking, exploring museums, reading, trying new restaurants, and traveling.

Dr. Hara Lam received her BS from University of Colorado at Boulder and her Doctorate of Optometry from Illinois College of Optometry. She is board certified and earned her Colorado Optometry License in 2016 and has provided eye care at various locations throughout Denver since then. Dr. Lam has worked with various patients, ranging from veterans with ocular diseases at the William Chappell Jr. Veterans’ Administration Clinic to pediatrics with binocular vision disorders at a local pediatric clinic. Dr. Lam’s special interests include fitting conventional and specialty contact lenses and management and treatment of corneal diseases such as dry eyes. Dr. Lam always tries to put herself in her patients’ shoes in order to make sure that their visit is comfortable and inviting. She enjoys sharing her professional advice and knowledge to patients in order to help them preserve vision and improve eye health. She is excited to work with the latest, state-of-the-art technology to improve patient care. Dr. Lam enjoys learning so she continues to study on the latest research about eye diseases and treatment. Besides being passionate about eyes and vision, she loves food, from eating to cooking. Dr. Lam also likes to bike and hike to enjoy beautiful Colorado. She also appreciates a challenge and enjoys piecing together puzzles and Legos in her free time.

Dr. Lam attended the University of South Florida, where she graduated with a Bachelor of Science in biomedical sciences, magna cum laude. She went on to earn her Doctor of Optometry with clinical honors from Pennsylvania College of Optometry at Salus University. Her experience in eye care led Dr. Lam to specialize in the diagnosis, treatment, and management of ocular surface diseases. She has also developed an enthusiasm for helping patients with severe dry eye caused by meibomian gland dysfunction find relief through innovative procedures. Dr. Lam has a passion for listening to her patients, understanding their needs, and educating them on their eye health. She provides exceptional optometry services, including dry eye treatment, myopia management, eye disease management, and comprehensive eye exams, and always makes sure patients are comfortable and confident in her care. Born and raised in Vietnam, Dr. Lam moved to Sacramento as a young girl and has lived in several other areas of the U.S. Through her travels, she’s developed an enthusiasm for learning about other cultures and is bilingual in English and Vietnamese. Dr. Lam now lives in Jacksonville with her husband, their two cats, and their puppy. And when she’s not helping her patients, she loves spending time with her family, learning new recipes, going to the beach, and exploring the city!

Dr. Lam is a local optometrist raised in McDonough, GA. He provides comprehensive eye exams for adults and children, diabetic eye exams, contact lens fittings, and ocular disease management to help patients live their best lives. Outside of helping the community, he enjoys playing tennis, hanging out with friends and family, and traveling.
